Complaint Form

In order for us to investigate the matter, please complete the following steps:

1. Complete the Identity Theft Victim’s Complaint and Affidavit Form (the “Complaint Form”) below. This Complaint Form will need to be signed and dated in the presence of a law enforcement office, a notary or a witness.

2. Take the Complaint Form and supporting documentation to your local law enforcement office and file it with them as a report.

3. Provide us with copies of your completed Complaint Form, any supporting documentation, and proof of your police report.

Your account will be placed on hold for thirty (30) days to allow you time to complete and provide copies of these documents. Upon receiving your documents, we will investigate your claim. If your claim is determined to be valid, the loan balance will be written off, any ACH authorizations will be terminated, and your account will be closed to prevent any further attempts to issue a loan using your information. However, if you do not provide the requested documentation during this period, your account will be deemed to be valid and collection efforts will resume.
